Limitation of my current Photoshop and other programs: can not open JP2 files. Tried to install plug-in from http://www.fnordware.com/j2k/, didn't work.
Instead, spending $69 to buy PaintShop Pro X5, which I can use as a bridge between Photoshop & JP2 (I hope).

Up early, awakened by Simon the cuddle-seeking (at 5:30 AM every morning) cat. Have been having Internet stability problems on the large computer in my studio. Moved the tiny wireless recepter, hope that helps.
There's a person called aibek in Internet Archives, who, although he called me out nastily once, telling onliners to "ignore Philly-Bob ...", he is very knowledgeable.
Some nuggets:
- If Google is annoying to download from, copy the url of the EPUB link, delete the ‘.epub’ and substitute ‘.pdf’ to get the link to the PDF link. Similarly, put ‘.djvu’ for ‘.epub’ to get the link to the DJVU file.
- For Google Books (uploaded by ‘tpb’) the PDF file is not searchable, but the DjVu file is.
- Everyone must have noticed that hardbound books are bound in a different manner than papercovers. Pdfbook produces prints which can be bound in the hardbound book style.
The whole book is divided into a few sections (you can choose the number), each section is to be sewn separately and then all of them sewn together. The sewing is through the middle of the folded page.
I have printed and sewn many IA books myself in the regular (bad) style. Here, sewing is done around half-a-centimeter from one of the edge of the collected pages. This takes up some of the “valuable real estate” of the page, and the pages do not open “completely” (you have to use both hands to keep the book open). In the hardbound style you get all the space, and when you open the book, the book will remain open! I recently printed a book in this style, and I am not going back to the old style! (A further benefit is that you need to sew only half the number of pages in the hardbound style compared to the regular style. And this sewing is easier too.)
In using Pdfbook, if you want the book pages to be A4, you would have to print on A3 size paper (and this would be folded.)
The number of pages per section is called “signature”. You can use Pdfbook to organize pages for right edge binding (Arabic, etc) too.
The software is part of the `Pdfjam' package. There is a `psbook' too which is supposed to work in a similar manner.

- Have you come across the DIY Book Scanning site? If you can do some tinkering, it would be trivial to scan in the manner mentioned there.
http://diybookscanner.org/
- Try Heiko Oberdiek’s ‘pdfcrop’ before printing the Internet Archive PDF files. It does a wonderful job of cutting out excessive white margins (automatically). This makes the fonts larger, and the pages much more attractive.

... You can also try another ‘pdfcrop’, and ‘pdfnup’. (See the links.)
